[ ] Hi it seems you have developed fissure in ano. ( a cut in anal canal due to passage of hard stool. ). You need to take laxative to soften the stool so that to pass easily without hurting the area. You need to do sitz bath( Sitting in hot water tub. It takes time to heal. Mostly can be managed with non surgical approach. There are certain medicines to relax the sphincter which aids in healing of fissure. Feel free to ask any further quary.

Pain is unusual in piles(hemorrhoids) unless the is an acute blood clot . It's possible that you may be suffering from a fissure. Hemorrhoids can be treated with sclerotherapy or rubber band ligation in most cases and is an outpatient procedure. Fissures can be treated medically with topical painkillers.
Next Steps
Sitting in a basin of warm water for 10 mins twice a day. Plenty of fluids and fiber (fruits and vegetables or an over the counter supplement like isabgol. can continue with the duphalac.
Health Tips
Using Indian closet or a small stool to keep the hip flexed while using a European closet. the angle between your trunk and thighs should be around 35 degrees
