Persistent Oral White Lines and Burning
Hello Doctor, I am 28 years old and I have been experiencing a persistent white line/patch-like appearance inside my mouth (inner lips and cheeks) along with mild burning and occasional tingling sensation. There are no ulcers, no fever, and no major systemic symptoms. My vitamin D was previously low, and I have completed supplementation. Other reports are mostly normal, but the oral mucosa is not returning to normal. Kindly advise what this could be and whether any further evaluation or treatment is needed.

Hello, Thank you for describing your symptoms clearly. A persistent white line/patch inside the cheeks and lips with mild burning sensation is most commonly seen in conditions like: • Oral Lichen Planus (immune-related condition) • Frictional keratosis (cheek biting/constant irritation) • Less commonly, a chronic fungal infection Since your lesion has persisted despite vitamin correction and is associated with burning, it should not be ignored. A few important clinical points: ✔ If the white pattern is present on both sides in a similar (symmetrical) manner, Oral Lichen Planus becomes more likely. ✔ If it corresponds exactly to the bite line, it may be frictional. ✔ If it does not wipe off and persists beyond 2–3 weeks, clinical evaluation is necessary. Most such conditions are manageable with proper diagnosis and medication.
Next Steps
I would recommend a proper oral examination (even via detailed online consultation with clear images initially). Early evaluation helps prevent complications and gives faster relief. You may connect with me online for further assessment and step-wise management guidance.
Health Tips
Avoid spicy, very hot, or acidic foods • Avoid tobacco (if applicable) • Maintain good oral hygiene • Reduce stress levels

Persistent white patches inside the cheeks and lips with mild burning may be due to conditions like oral lichen planus, frictional keratosis, or less commonly a fungal infection. Since it has not resolved, a proper clinical examination is important. I would recommend an in-person evaluation, and if needed, further tests such as scraping or biopsy to confirm the diagnosis and start appropriate treatment. Early assessment is always advisable.
