My penis shape is like mushroom . Penis urethra opening is just below head of penis there is no difficulty in intercourse and fully inserted in vagina and orgasm also .I have tested semen also all reports are normal .is it OK to concieve baby .ma so worried about this last 1 year trying for baby still no +ve result .is it enough orgasm inside vagina or semen should reach cervix?

It seems you have a distal hypospadias, ideally deposition of semen in vagina is enough for pregnancy, but in case you still feel to get evaluated consult an Urologist.
