Hello mam. The tooth seems to be fine in the x-ray but the problem is with the supporting structure of the tooth. The x-ray shows bone loss on one side of tooth extending upto the root apices. It might be due to prolonged food lodgment.
Next Steps
Scaling and root planing followed by eliminating the cause of food lodgment, localised flap surgery with bone grafting is needed. Root canal may also be needed.
Health Tips
Please visit nearby dentist to get treated before you get pregnant. Leaving the tooth unattended at this time may lead to more problem in future.

Hello, From picture it's clear that it's a periapical infection because of which you having pain. The swelling is because of the pus formation. As you experiencing pain and the pus is there most likely you must have been prescribed some medications so please continue with it ,then the RCT procedure with capping will follow. If you planning to convince it's better to get a full oral examination from cavities to cleaning all of it because no oral treatment is recommended at time of pregnancy until and unless it's emergency it can be done in second trimester. The medicine you take while treatment of it your tooth will be normal antibiotics and analgesic and it's completely ok to take them if you planning a pregnancy.
Health Tips
Do not use hot packs or eat hot and hard food as it might increase your pain..

From the x-ray, looks like the tooth is infected internally, the origin of which could be because of decay or inflammation of the gum. As you are planning to conceive, at the moment a basic pain killer and low dose antibiotic should work.  If it is possible, get the tooth treated before you begin pregnancy.
Next Steps
Cleaning of the tooth and gums is essential to remove the infection
Health Tips
Please visit the dentist soon. Avoid extremely hard and hot food items.
