Pain exactly below left nipple 1mm below

I have a very small clot in my left nipple as i have gynecomastia for last 10 years and due to which there is very small mass type clot.6 months ago the clot/mass start paining while touching it.without touching there is not any pain.i dont have any cough,shorten of breath,no fever,no redness.but i have gynecomastia i show it to doctor 6 months ago but that time there is not any pain and he said it is naturally it automatically cure.but after few days i notice that there is pain and pain felt only while touching the area.the pain is only in 1mm area exactly below left nipple.the pain is very acute and pointed.after i stop gym the pain occurs earlier the clot is from last ten year there is not any pain.after death of my father i stop gym and after 1 months i have chest pain as i take lot of tea.its due to gastric problem now the chest pain is gone but the pain in nipple is still there.so plse can u suggst me some suggestion what should i do..or whether the pain is due to muscle strain
