Pain and itching
In my palm of right hand is very itchy and aching and mild swelling is also coming. There is a lot of pain in my shoulder as well of the left hand and it is not able to straighten

Yes i agree with earlier suggestions... You need to consult a dermatologist for the itchy palm. I am not sure if i understand you right about your right shoulder. If liting your shoulder forwards is painful with your pain being in the front of the shoulder, icing could help. Both your problems could be be of different origin.
