2. Gabapentin, alpha lipoic acid & mecobalamin tablet
3. Flupirtine Maleate Sustained Release Tablets
4. Tolperitas hydrochloride and diclofenac sodium tablets
5. Paracetamol Tablets

Merely report can not help me to identify problem, unless patient tell me the complaints to correlate clinically.
But according to MRI this patient might have low back pain with both lower limb radicular pain.
And for such case, after evaluating MRI films the plan for percutaneous Disc Procedure can be executed with better outcomes.
