Need physician statement for insurance
I have applied for a term insurance and declared that long back I use to have breathing problems but now it is very rare like once in 2-3 months.
For this I need a doctor statement filling the form thay gave me.
I would like to know where I can get this filled and what test needs to be done if any.
They said any MBBS doctor can do it.
I am attaching the form for reference
