Need a nedical certificate form 10 IA
Hi Dr
My mother aged 80 years is a parkison patient with stiffness leading to fall and injury.
Sh e has undergone DBS surgery in 2010 and condition is better( medication lowered).
Currently she is better however from Income tax perspective need a letter in specific format for 10 IA with signature.
I have form and past consultation papers as well.
Can you pls help with the certificate.
