For the nail discoloration, since it has been persistent and has turned brownish-black in one finger, it’s important to get it examined by a dermatologist to rule out fungal infection or, rarely, other causes.
Regarding the shoulder and arm pain, since it is persisting despite medications, worsening at night, and now affecting both arms, it may need further evaluation with imaging like X-ray/MRI or a neurologist or orthopedic review to check for nerve compression or cervical spine issues. Meanwhile, avoid overuse, apply warm compresses, and continue gentle stretching as advised by your doctor.

Both your nail changes and persistent arm pain need in-person evaluation. Nail discoloration lasting months can have several causes and should be checked by a dermatologist; the arm pain needs an orthopedic/neurology exam with possible imaging. It can’t be confirmed online. If you’d like, I can guide you step by step after an in-person consult.
Next Steps
Dark discoloration in a single nail and ongoing arm pain need proper in-person evaluation. Nail changes could be infection or trauma, but rarely something more serious, so it’s best to have it examined by a dermatologist. Your arm pain also needs a physical check an orthopedic/neurology exam and maybe an X-ray/MRI to rule out cervical spondylosis or nerve impigement if posture correction through physiotherapy doesn’t help.
