Yesterday I visited an ENT for the 4 month long symptoms of pressure around left eye, behind left ear and left side of head
The doctor advised FSST+Septo+BTR surgery after looking at the MRI
Should I go for the surgery?
I have attached the prescription for reference.
Please suggest.

Maxillary polyp depending on size can be treated medically or surgically.
Hypertrophy of terbinate can also be treated medically and surgically (surgically gives better relief).
Deviation of nasal septum with spur can only be treated surgically.
So to conclude you need surgery.
