If Micrometastasis and isolated tumor cell from breast cancer  present in sentinel lymph node biopsy .why it is not considered as a metastatic disease and  why treatment or stage dosent changed nased on that finding..isolated tumor cell <200 cell and <]0.2 mm ..micrometastasis is >200 cell size may be more than 0.2 mm but less than 2 mm

A micrometastasis( pN1mi) to axillary lymph nodes metastasis will not fall in the category of metastasis as per TNM Staging for breast cancer. Stage 1B. We usually call axillary lymph nodes metastasis if it is macroscopic or >200 cells. Metastasis  means not always stage 4 unless it had distant metastasis ( example cancer spread to liver, lung, bone)
