A scrotal Doppler won't detect obstruction but a transrectal ultrasound can. Since fructose is present the chances of obstruction are less. The parameters of seminal analysis are deranged to a significant degree.
Next Steps
can consider artificial insemination with donor sperm, can also consider testicular biopsy at a specialized fertility centre so that if a viable normal sperm is found it can be used for fertility .
