As report suggested there is major issue of osteoporosis Though patient age was not given but I can guess patient is in 50 or above age So it can be senile osteoporosis Need major dietary supplements to restore vit D 3 level and rich Calcium source in diet Degenrative  changes also suggesting same

As per the report, there is deviation of normal curvature of spine towards lateral side. There are degenerative changes present in lumbar spine. Also, there are changes related to osteoporosis.
Next Steps
You should start with hot fomentation,  apply Lumbosacral belt for support,  take analgesic with muscle relaxant,  calcium and vitamin d supplement,  apply analgesic ointment locally. MRI would be better investigation to find the cause and severity of the problem.
