Is it copd or asthma
Hello doctor I am 25 and have been having extreme sob from past 1 month..I have asthma since childhood ,doctor changed my dosage also but still bronchodilator doesn't give relief..X-ray hrct were normal..my Spirometry is abnormal..I am non smoker for entire life and even dint come across passive smoking..I am mbbs student..my question is can I have copd...should I get alpha1antitrypsin tested...my oxygen saturation is normal...but sob doesn't go away..sharing my pft report please give your feedback

Firstly, childhood history of breathlessness with diurnal variation (if present) is suggesting bronchial asthma only. If from childhood itself the management hasnt been optimised will cause asthma with fixed airways, thereby showing the current spirometry values here. It can be difficult to treat asthma category and will have to start on formoterol budesonide MDI 2 puffs daily provided the technique is right. If your technique is right and you are already on high dose inhalers, you may proceed for an HRCT to rule out other causes of asthma such as ABPA.

MDI formonide 200mcg 2 puffs BD x 3 months T. acebrophylline 100mg BD x 2 weeks repeat spirometry after 1 month

The PFT report is not visible clearly. Though the loop appears to be poorly performed. What medications are you on? Just looks like poorly controlled asthma to me. You should be evaluated thoroughly
