Is it a fissure or anything else

Asked for Male, 26 Years

Hello , I recently showed to a surgeon he just inserted lubricated finger glowes into anus and ask for any burining sensation or not but thier was nothing such. But i get this problem atleast once in a week . The attached picture is after washroom when it was red and afterward on application of cremagel it looks like it went . Please just me proper treatment.
