29M, 98kg, 178cm (BMI 31). Fasting labs 22/06/2026. Need cardiologist opinion. History: Hypertension (on Cilnidipine), chronic urticaria (on Cetirizine), Finasteride for hair loss. Scalp folliculitis active during blood draw. No diabetes. Lipids: TC 187, LDL 129, HDL 39 (low), TG 91, Non-HDL 148 (high), ApoB 97, Lp(a) <7 (normal) Cardiac risk: hsCRP 3.22 (folliculitis active), Homocysteine 25.7 (high) Insulin: FBS 93, HbA1c 5.7, Fasting Insulin 54 (ref 2.6-24.9), HOMA-IR 6.4 (ref 0.7-2.0) Other: ALT 64, GGT 98 (possible fatty liver), Vit D 16 (deficient). Kidney, thyroid normal. On methylated B-complex for homocysteine. Questions: With HTN + insulin resistance + ApoB 97 + LDL 129 at 29, start statin/ezetimibe now or lifestyle first? Is Cilnidipine alone adequate for my profile? Need echo/calcium score at my age? Started my weight loss journey. Am i right candidate for glp1?
