I need a medical certificate. Me doctor.

I got operated for blood clot in brain in 2015 June and was under medications till 2017 Dec. I began my work in June of 2017 and had on and off nail striking hammer headaches bcoz of work stress. I was sexually assaulted one night when I had severe headache with vomit by my workmate. He had that video and he used me whenever he wants threatening me. I got pregnant that month itself but bcoz of my health issues doctors refused to do abortion. I am an orphan nad had lots of heath issues during delivery and it took almost a year to recover. I filed compliant against him and had many and now NCW has come forward to help me. My baby is 2 yrs old. I am in need of a medical certificate about my bad health condition to punish that paychoraper. I was mentally affected in these 2 years bcoz of incident and not sleeping at all in the night time. Please help me doctor.
