While doing penetrative sex foreskin can get ruptured or torn if its very tight leading to bleeding and pain.
It may not happen in masturbation as you may not force skin forcebly.
Please go and get examined by surgeon for tight foreskin. If its there, you may require small surgery called as CIRCUMCISION.
