Small size of ECG complexes in lead II may be a sign of an underlying heart disease and sometimes this may just be normal physiological change. A proper history and clinical examination will be the key step in evaluating your ECG issue. This may be followed up with further necessary investigations. So please meet a cardiologist as early as possible and get yourself personally examined for a final advice on this issue. Thanks. Hope this helps.
