High sexual desire

I am not able to control my sexual desire. Without doing anything it pops up any times in a day till I mastrubate and I feel completely helpless. It's so compelling that I can go through any extent to do mastrubation. I tried yoga also but no help. even I tried no fap since 4 months now but I am relapsing in 6 days maximum. during no fap I feel so bad with high urges and my chest become so heavy. Please help with this. If there is any medication that I can take. is there any test can be done to check my reason of my urges. is it mastrubation addiction? I have done testesrone test will attached the report here.
