Hyperlipidemia SX – claudication, angina, MI, stroke, pancreatitis, chylomicronemia (Trig > 2000), atherosclerosis. PE – xanthomas.
Next Steps
DX – • Fasting Lipid Profile: Chol > 200 | LDL > 100 | Trigs > 150. • Also: CBC, CMP, U/A, TSH/FT4, LFTs, CPK.
Health Tips
TX – diet, exercise, no alcohol, no smoking. • Pravastatin 40mg QD. • Ezetimibe 10mg QD. • Ezetimibe/Atorvastatin 10/20 QPM. • Niacin 250mg QD. More specific for Hypertriglyceridemia: • Gemfibrozil 600mg BID. Fenofibrate 48mg or 145mg QD. Tx if patient has elevated LFTs: Ezetimibe 10mg QD. Cholestyramine start 4g QD. Dose 4-8g PO BID. Give before meals.
