Hello Doctor, I am currently pregnant and i recently had some test results and would like your opinion. HCV Antibody (CLIA): Reactive, value 1.33 (cutoff >1.0 = reactive) HCV Antibody (ELISA): Non-reactive, value 0.27 (cutoff <1.0 = non-reactive) HCV RNA (PCR, Real-Time): <35 IU/mL ALT (SGPT): 8 U/L (Ref: 0–31 U/L) AST (SGOT): 14 U/L (Ref: 0–32 U/L) Could you please clarify: Is this considered an active HCV infection? Should I repeat the HCV RNA test? Is treatment necessary based on these results? Thank you for your time and guidance.

You have done Anti HCV by two different methods with different results because of their sensitivity. Anti HCVis a screening test which detects antibodies for Hepatitis C. It can come as falsely positive, or may indicate remote infection. If HCV RNA is negative (as in your case), there is no need of anti- viral treatment.
