Headache over the back of your head (“suboccipital” region) that are severe and worsen with coughing or straining can be due to a Chiari malformation. Rarer causes of headache could tumors or cysts - and the chances of someone having that would depend on the presence of other symptoms and signs. In most cases, there is no sinister finding, and simple painkillers will suffice.
