Headache and neck pain. Months but no i!

Respected sir ,                     I'm 29 year old suffering from acute headache and neck pain from last 4 year                     headache is at temple side  but left side gives more pain and there is swallowness I feel so just near and above ear at both right and left side .                      neck is not flexible ,there is steefness  .while moving neck it gives pain .  and I feel  cracking like sound while moving neck .   only side portion of neck gives pain , there is no pain at mid portion .                                I have also shoulder pain but headache and neck pain made my life more complicated                                I can not remember,  my memory is weakened ,even what I read before 5 minutes I can't remember , also I feel heaviness of breathing   sudden waight is gained I was 42 kg before now I am 60 kg , heaviness of eyes , indigestion .                               so I went to neurologist  , they prescribed me some tablets I took upto 8 months but no i
