Frequent urination please suggest

Asked for Male, 28 Years

Hi . I am Male age 28 y, Experiencing frequent urination and mild continuous pain in bladder which comes and goes. Mild burning sensation when ejaculation. Got tested for urine and local urologist prescribed meds. Condition doesn't seem to get better and seems to getting worse Attaching reports and prescription What might be the underlying disease Please help.
