Foreskin problem from last few days

I'm an uncircumcised male 34 year old and from last few days, my foreskin looks swollen and pains on retracting.
Visited a skin doc who gave me ebernet cream and ketoconazole tablets.
It reduced red spots on penis but still feels very tight foreskin like it's swollen and pains.
Penis head also looks reddish than normal.
Got blood tested, no sugar no issues in it.
What am I suffering with.
Is it serious?
