Fitsula in ano

Is fitusla in ano a pre-malignant condition? My brother is suffering from fitsula in ano from 2 year but he is not letting us take him to surgery because of his fear. He is having sticky fluid discharge and sometimes blood from opening of fitsula. Kindly guide has his fitsula turned into Cancerous or pre cancer stage?
