First degree av block

I have diagnosed with CHB with 2nd degree av block in Sept 2022. I had no symptoms by then. Now I have chest pain extreme fatigue and easily get tired while walking. Doctor had suggested pacemaker of necessary? Whether I need peacemaker? Will this 2nd degree turns into complete block? Any risks of heart failure without symptoms?
