We've done series of investigations for the fever blood culture, urine culture, tb sputum test, hiv, hepatitis b  etc and finally pet ct which showed Pylonephritis with paraotic node and retrocaval node , aortocaval node enlargement what would be the next treatment as urine culture turned negative

Urine culture can be negative because of prior treatment with antibiotics Can repeat and see if still fevers persist and symptomatic If repeat urine culture is also negative, can check for 1. URINE AFB to rule out genito urinary TB 2. TRUS to look for prostatitis
