Feeling dizziness
I am suffering from multiple scelerosis and i am bed ridden from last 1 and half year. I am taking rituximab (once a year) and Methylprednisolone (once in a month). On 23 March i fell down from my bed while exercising and after that i am feeling dizziness while doing any movement. And even i get so much cold, cough and breathing problem..what should i do. Pls suggest me something and help me.

Mam, I can very well understand the physical and mental and emotional pain and despair that you are going through. May I have the privilege to try to help you ? For how long have you been diagnosed with multiple sclerosis ? Are you talking an annual dose of intravenous rituximab ? For how long ? In what dose are you taking methyl prednisolone monthly and for how long ? When you became bed ridden one and half years ago did you have loss of balance then or vertigo/dizziness ? Are you taking any other medications for any other diseases ? I assume the drugs you have mentioned were prescribed by a neurologist ? Let me tell you that as multiple myeloma progresses there might be loss of balance, vertigo and dizziness. It could be due to the affection of the vestibular nerve of the inner ears which in connection with the cerebellum maintain our balance. You will also be surprised to know that rituximab and methyl prednisolone may cause dizziness. However since you are taking these yearly and monthly respectively, it's more likely that the culprit is multiple myeloma which may affect the cerebellum too, the seat of balance. Beware of methyl prednisolone ( powerful steroid ) induced secondary diabetes mellitus which can cause repeated respiratory tract viral/bacterial infections causing coughs and colds frequently. So do a fasting and PP blood sugar test with HbA1c too and let me know the report. Methyl prednisolone will also cause lower immunity so that the chances of various types of infection is more in your case, tuberculosis etc is common in immunocompromised patients. Are you taking intravenous solumedrol monthly or Medrol/predmet tablet ? Also a doctor should check your lungs to detect any rales and bronchospasm and blood pressure too. Physiotherapy and exercises are important for you. But for dizziness there are many vestibular exercises which is done by manoeuvring your head into different positions and moving your eyeballs/closing your eyes etc. I think this will help you. Do it for 45 minutes daily. You will get an illustrated pamphlet in the Ent dept of a hospital or via net. Infact a Ent consultation at home is welcome apart from a neurological review. Also have your blood pressure checked along with a blood sodium and potassium as electrolytes disturbance can cause weakness and dizziness. Methyl prednisolone can itself cause shortness of breath, high blood pressure, potassium disbalance apart from the blunted immune response. You are already having breathlessness. You can yourself understand when methyl prednisolone takes it's toll upon you. You will gain watery weight, your body fat distribution will change, a hump on the back of your neck, a protruding belly with thin arms and legs, called lemon on a match stick. Any psychological change induced by the potent steroid ?
Next Steps
Make sure you are not off balance when you get up from a lying position in bed. Get up slowly. Take care of your low back region by gentle cleansing, oil massage and frequent change of posture while lying down so that your backside is not subject to continuous pressure at a same point for more than half hour. This will prevent pressure sores. Exercise of torso, limbs etc and physiotherapy to continue.

Provided you don't have allergies you may try: 1. Tab Vertin 16mg: 1 tab 3 times daily for 15 days or Vertin sr 48 once daily. 2. Tab Nervup OD: 1 cap daily for 3 months. This is a nerve vitamin. Please ask your neurologist to review you and your medicines along with an ENT consultation. Do the vestibular exercises. I don't know whether you are still able to walk, if yes then always use a walker. I would be very glad if my advice and explanation provided you any help.
