Fatty liver with 9.7kPa stiffness
OBSERVATIONS: Liver fat quantification showed the fat fraction of 7.6%, suggesting grade – 1 steatosis. Elastography performed showed widening and slow progression of shear waves. Stiffness maps showed the stiffness to be around 9.7 kPa, suggesting stage – 4 fibrosis. Hepatic iron concentration is 15.5µmols/g suggesting no iron overload. Mild hepatomegaly is noted with diffuse surface nodularity. No focal lesion. Gall bladder is distended. minimal sludge is noted within the lumen. No biliary dilatation. Spleen is enlarged in size and measures 14 cms supero-inferiorly. Pancreas is normal. Impression:  Stage – 4 hepatic fibrosis.  Grade – 1 hepatic steatosis.  No evidence of iron overload. Currently taking medicine -udiliv, omega and uric acid tablets. MRI also was done 1 and half years back where liver stiffness was 5> and fact friction was 9.8% Please suggest next treatment.
