Dandruff and hairfall

I tried many treatments for dandruff problem but not getting any benefit
I have used ketoconazole shampoo it gave relief but after some time dandruff again came back
Then went to dermatologist doctor
He gave onabet sd lotion(2 times for 30 days), 8x shampoo, cetcip L tablet for 30 days
The treatment gave relief initially but after some time dandruff again came back
Then again went to same doctor
He gave zydip c lotion (2 times for 45 days and 1 time for another 45 days), 8x shampoo for 3 months
Again the treatment gave relief but same dandruff again came back (this time with heavy hairfall also)
Pls advice something...
