I have samall cuts on penis tip and whenever it heal it makes my tip hole smaller and now m not able to open my foreskin because if I open it hurts and cuts again

Considering your age, it is best to meet a medical practitioner and have a complete examination to rule out causes like infection or any chronic condition.
