My grandmother,85 yr old,k/c/o HTN,DM,IHD,CKD is on Tab Amlo & Tab Tormesimide 10 mg each OD,Tab Glimy 1 mg OD,Cap Ecosprin 75mg OD,Tab Prazosin OD. Day B4 yesterday,she became drowsy & she went into hypoglycemia. V did sugar testing every 2 hourly Wich came up to 36,43,74 inspite of gvng lots of sugar. She had take all her tablets day B4 yesterday.Yesterday v did sugar testing every 2 hourly with additional sugar,have not given tablet Glimy yesterday...Her reading have progressed to 106,137,193,225.Today we have stopped giving sugars to her,B4 breakfast i.e empty stomach sugar was 100,post breakfast it was 225. B4 lunch it was 126,now we will again do at 4:30. I m a physiotherapist by profession so monitoring everything...Can u please help me & tell me what shud u do?? V stay at Dombivli & none of the doctors are ready to take up the case. Thank you

At the age of 85 years low sugars  is more dangerous than high sugars, get her hba1c tested, if high she needs treatment with drugs with better safety in ppl with kidnEY  problems... stop glimiperide for now

Ur grandmother doesn't need strict control of sugars because of her age and comorbidities..for CKD patients glimy is not advisable..she needs more short acting tablets..her target sugars should be 140-180 mg/dl

Hello, She has to take proper diet as advised by doctor. If she is not taking food you stop glimy 1mg at that time because it will cause hypoglycemia. She can continue other medications as per advice. She needs complete evolution in hospital.

Urine routine and ketone should be done,FBS AND PPBS ,HBA1c .

Get hba1c done... If within normal limits... No need for glimy or insulin... Still daily sugar levels not under control and If difficulty in using insulin... Tenepride 20 can be used... It has good renal safety and no chances of hypoglycemia...

In view of her CKD, I wouldn't want to control sugars with glimy .. insulin would be better. Get her tests done for CBC, urine complete,RFT With Sr. Electrolytes.

In presence of CKD, glimepride should not be taken. Hence stop it, monitor blood sugars without any antidiabetics and if found raised, control with insulin.
