Constipation and anal fissure
I had anal fissure diagnosed in november2020 due to  stool blockage and loose motions.Then after i managed anal fissure with ointment and laxatives.I managed 4months like this. I thought that fissure had healed.But after 4months the fissure come back and start causing pain.Now also from last 1 month iam suffering from constipation.my motion also not coming,it comes like day after day and also l have to put more pressure while pooping .I don't know whether it is  happened because of fissure or constipation. Please help me to get rid out of this. iam also using ppi's for gerd and hiatus hernia

You have fissure because of constipation..if it chronic fissure it won't heal with conservatives methods. Need lateral sphincterotomy..along with life style modification.
