Concern About Elevated Uric Acid and SGO
Hi Doctor, One month ago, my SGOT was 28 and SGPT 47. Now SGOT is 49 and SGPT 32. I was doing a lot of intermittent fasting and recently ate only veg soup with moong dal. Three days before the test, I ate chicken steak with a little rice and veggies. The next day I had loose motion. I also started weightlifting again after 1 year just a few days ago. For the past 10–12 days, I’ve been taking milk thistle twice daily. In my recent tests, KFT was normal but uric acid was 8.7 SGOT 49 lab normal value <40 . My doctor prescribed Febuxostat 80mg for 2 months. Is it necessary to take this medicine? Could fasting, ketosis, diet, or muscle recovery raise uric acid and SGOT? because my body start making keton bodies,I also have mild calf pain muscle pain,and another problem is taking time to fall in  sleeping. I’m 24, living away from home, and have a history of post-COVID drug side effects, so I’m worried about taking strong meds again. Please guide me on what to do and eat.

Increase in uric acid is due to ATP turnover (Workout) 2) Interetent fasting and Ketosis causes: Rise in ketone bodies and uric acid also reduced uric acid excretion. 3) High protein diet: protein makes purine and purine makes uric acid. 4) Loose motions: Loss of fluid concentrates uric acid in blood. A) Febuxostat is a strong uric acid-lowering drug, mainly for gout, tophi, or recurrent uric acid kidney stones. Not the first choice for someone with asymptomatic high uric acid — especially at your age and with no gout attacks.

DO NOT TAKE FEBUXOSTAT RIGHT NOW. (Uric acid elevation is likely transient) 1) Stay well hydrated (More than 3 Liters per day) 2) Avoid excessive protein all at once. 3) Moderate intake of Dal and Chicken. 4) Avoid Red meat, Sugary drinks. 5) Add the following into diet: Cherries, Amla, Lemon Water, Whole grains. 6) Add multivitamin suppliment spicially B12 and D3, Add Magnesium glycinate 200mg at night for better sleep and reduced muscle pain. 7) Cold shower before bed. Repeat following test after a month: Uric acid SGOT, SGPT Vitamin D, B12, Creatinine

Uric acid a cause for gout. Febuxtare 40 mg for one month  followed by 20 mg for second month. Change life style n diet plan drastically.  As such not too much high liver enzyme.
