Purely based on MELD the mortality would be nearly 10% at 3 months. But you have to consider other variables, such aş ascites and high risk varices. Besides, any patient with MELD more than 15 will likely benefit from a Liver Transplant, especially in the presence of multiple decompensations such aş Ascites, jaundice, Hepatoc encephalopathy

There are multiple factors which are involved in this scenario. Patient's comorbidities and age also play an important role. However it is estimated that without transplant 50%mortality can be seen in 2 yrs time.
Next Steps
if the patient is otherwise good, he should be assessed for Liver transplant Meet a liver transplant team for the same
