Chlamydia culture and sensitivity. It?

Where near Bulandshahr, UP I can get clamydia culture sensitivity test done and get treatment for it? I am unable to find any lab. I had got DFA done in Safdarjung hospital 3 years ago which came positive. I was given doxycycline for 21 days after which my symptoms disappeared and there was no sexual history. However, non sticky with no smell, milky white like discharge (only in morning) never stopped. I strongly feel chlamydia was not cured completely as I have got chronic epidymitis, proctitis, intermittent burning sensation while urinating. Also, my new partner has also shown symptoms just after aur first physical contact. Need checkup (more important as azythromyin, levofloxin didn't help me last time when prescribed) and treatment near Bulandshahr, UP
