One can have chest pain due to blockage in coronary artery but only pain (Angina) won’t cause increased Troponin or CKMB. Heart attack is Myocardial Infarction which means heart muscle is damaged (dead or necrosis) due to lack of blood supply. In that condition cardiac enzymes get released from the heart muscle cells into the blood. So normal enzymes rule out heart attack but not Coronary artery Disease.
