Will need to see CAG, if good target vessels ideally CABG. If known case of CKD prior to MI and CAG has not worsened Urea and Creat values (CIN). CKD status will remain unchanged after CABG . May need dialysis (SLED) or CRRT. LRTI needs to be addressed with suitably modified(renal) antibiotics
