I am a 16 yr old male having big dark brown puffed nipple skin . Its not paining but its making me feel uncomfortable. Whenever i touch it , it shrinks and turns to a small dark nipple which is ok.

This reaction is normal.  Nothing to worry. If u r unhappy with large size of nipple then it can be surgically corrected.  Or else Nothing to be done.  Warm regards Dr Govind Suresh Mittal, Bengaluru.
