Blood in urine and pain while urination

Asked for Male, 31 Years

I am having pain while urinating and blood and clot comes at the end of the pee. Will semen also have blood ? I have got my ultrasound and renal profile 3 months ago and the reports were normal. Then What is the reason for blood in urine.
