Baby's persistent diaper rash

Asked for Male, 1 Years

My baby will be 3 months in 8 days and he's having a diaper rash that's getting worse regardless of the diaper rash creams that I've been using on him. He also poos very often and it's like diahrea. It's been 3 days with the rash and won't improve. It's very red and he bleeds from tiny openings in his skin. When it dries, it feels rough and hard.
