Appendicitis with mass formation.
My father was treated after appendicitis with mass formation 10 days back with conservative treatment with 5 days admitted in hospital.  The ultrasound taken today and 10 dats back shows reduction in size from 9-11 mm to 6-7 mm. But todays scan shows 9x7 mm sized peri appendicular collection with marginal omental fat seen in right ooliac fossa. I consulted two surgeons. One surgeon suggested to start IVR based treatment in hospital and told 9x7 mm size could be pus so told to do CT scan where as other surgeon told unless there is pain dont do anything just continue tablets and do scan after 4 weeks for operation if pain appears. As of now there is no pain whatsoever. I am in dilemma what to do.

Both the Possibilities are correct. Collection can be some simple collection or some pus collection. Ct scan can help in making the decision although even that is sometimes not confirmatory. The decision based on SYMPTOMS like no fever or improving GENERAL signs are also helpful in deciding. This decision will always remain a dilemma even for a Surgeon. The only way to move ahead in th is scenario is to get a ct scan. So go for it.
