Hello, X-ray is required to diagnose ,he might have had trauma , in uncontrolled diabetes person will have ankle edema , not ankle pain . Get xay of ankle and consult orthopedic surgeon in nearest hospital. His blood suger level should be checked once , if it's shooted up necessary measures will be taken to control diabetes .
