Your chief complaint is pain the lower right jaw because of your impacted third molar.
That should be addressed first.
But as your oral surgeon said, upper ones need to be removed too..that is because our teeth are equally placed in the jaws..upper and lower. There is always a tooth in the lower jaw to counter another one in the upper. When the lower third molar is removed, the upper third molar will start drifting down into your mouth which is termed as supra-eruption.
On the radiograph your upper third molar isn't straight, it's positioned to the right side into your cheek..and when that starts to come, it'll start hitting your cheek, hence it would be good to take it out in one appointment.
And about your concern of getting all 4 removed..speak to your surgeon and do one side at a time.
Upper and lower right side first as it is your complaint..
And later left side.
