Hello there! According to the symptoms you have reported, there are three possibilities,
1. The gums around it may have become inflamed and there may be pus formation due to food impaction in its surrounding gums.
2. The wisdom tooth may have already developed caries(cavity) if there was food impaction since a long time. This cavity maybe the cause of pain if it has reached the nerve(pulp) of the tooth.
3. One rare possibility is a deep cavity in the tooth preceding your wisdom tooth which may have formed because of food impaction in between the second molar and the wisdom tooth.
Nonetheless, a sound diagnosis and treatment plan depends on clinical examination and X-RAY of the affected region.
The treatment does not necessarily need removal of the tooth. Apart from tooth removal, reshaping of the gums around the tooth may help if we are facing the above mentioned 1st possibility.
