Seeing d patch and its since 1.5 yrs and increasing in size my advice will be go for incisions biopsy for confirmation diagnosis . Seems to be oral leukoplakia. Can cosult .

As per your history of smoking and crak appearence over the tongue I suspect Leukoplakia associated to Candidiasis. Stop all form of tobacco . I advice tongue scraping habits when ever you brush. - Tab ClenoRush Troche 10mg Chewable tablets 1 tab od x 10 days - Candid Mouth paint , apply over the tongue after cleaning it with a cotton ball. - Cap LycoRed  1 Cap  od x 15 days. This picture is not enough to diagnose the lesion as cancerous or not. Are you experiencing any other symptoms? Like burning sensation every time you eat anything, or reduced mouth opening, or pain or any drastic weight loss history? This apart, since you said it is growing in size, it is better that you get a biopsy done and get a confirmed diagnosis.
