Upper back pain

I had upper back pain for the last 8-9 months. Took three dry needling sessions to release trigger points pain was reduced  to 60% and was having  a consistent pain , when I am getting physiotherapy pain is less when I stop pain increases. Following are my MRI findings Screening MRI Whole spine findings reveal. Mild spondylodiscal degenerative disease as described. C3-4 diffuse posterior bulge causing mild indentation on anterior thecal sac. L4-5 diffuse annular bulge causing mild indentation on thecal sac and mild encroachment of bilateral neural exit foraminae, with mild compromise of bilateral exiting L4 nerve roots. L5-S1 diffuse annular bulge causing effacement of epidural fat, mild indentation on thecal sac and mild encroachment of bilateral neural exit foraminae (right>left), with mild compromise of bilateral exiting L5 nerve roots. Can anyone tell what's the reason for this pain ?

In simple words, your MRI report shows: 1. Mild wear and tear in the spine discs. 2. At the level of C3-4, there's a bulging disc that slightly presses on the sac surrounding the spinal cord. 3. At L4-5, there's another bulging disc that slightly presses on the sac and also narrows the openings where nerves exit, which might be affecting the L4 nerves. 4. At L5-S1, there's a bulging disc that presses on the sac and narrows the nerve exit openings, which could be affecting both L5 nerves, more so on the right side.
